Description

This measure is used to assess the proportion of patients with acute leukaemia undergoing treatment with curative intent who die in first complete remission (CR), within 1 year of diagnosis.

Note from the National Quality Measures Clearinghouse: This measure is part of the Cancer Quality Performance Indicators (QPIs) collection. For more information, including a complete list of QPI measure sets, please visit the Healthcare Improvement Scotland Web site External Web Site Policy.

Rationale

Outcomes of treatment, including treatment related mortality, should be regularly assessed.

This cancer quality performance indicator (QPI) measures the quality of supportive care provision and management of complications in patients treated with curative intent who achieve morphological remission following consolidation therapy.

Evidence for Rationale

NHS Scotland, Scottish Cancer Taskforce, National Cancer Quality Steering Group. Acute leukaemia clinical quality performance indicators. Edinburgh (Scotland): Healthcare Improvement Scotland; 2016 Jun. 30 p. [19 references]

Primary Health Components

Acute leukaemia; treatment with curative intent; complete remission (CR); death

Denominator Description

All patients with acute leukaemia undergoing treatment with curative intent who achieve first complete remission (CR) (see the related "Denominator Inclusions/Exclusions" field)

Numerator Description

Number of patients with acute leukaemia undergoing treatment with curative intent who achieve first complete remission (CR) and die within 1 year of diagnosis, whilst in CR (see the related "Numerator Inclusions/Exclusions" field)

Type of Evidence Supporting the Criterion of Quality for the Measure

  • A clinical practice guideline or other peer-reviewed synthesis of the clinical research evidence
  • A formal consensus procedure, involving experts in relevant clinical, methodological, public health and organizational sciences

Additional Information Supporting Need for the Measure

Unspecified

Extent of Measure Testing

The collection of data is piloted on a small number of patient records using a paper data collection form produced by the Information Services Division (ISD). The aim is to identify any anomalies or difficulties with data collection prior to full implementation. At least one NHS board in each Regional Cancer Network participates in the pilot.

Inclusions
All patients with acute leukaemia undergoing treatment with curative intent who achieve first complete remission (CR)*

*Within the measurement of this cancer quality performance indicator (QPI) complete remission as confirmed by morphology will be utilised.

Exclusions
Patients undergoing bone marrow/stem cell transplant

Exclusions/Exceptions

Medical factors addressed

Numerator Inclusions/Exclusions

Inclusions
Number of patients with acute leukaemia undergoing treatment with curative intent who achieve first complete remission (CR) and die within 1 year of diagnosis, whilst in CR

Note: In order to ensure that a full 12 month period has elapsed since diagnosis, enabling accurate measurement, this cancer quality performance indicator (QPI) will be reported 1 year in arrears. This will ensure accurate and appropriate reporting against this QPI.

Exclusions
Patients undergoing bone marrow/stem cell transplant

Prescriptive standard

Prescriptive Standard

Target:

  • Patients under 16 years of age: less than 4%
  • Patients 16 years of age and over: less than 10%

Target level is stratified by age as due to intensity of treatment risk of complication increases with age.

Note: Varying evidence exists regarding the most appropriate target level; therefore, this may need redefined in the future, to take account of new evidence or as further data becomes available.
